One study looked at genetic risk areas found through genome-wide association studies (GWAS). These studies compare the genes of people with and without a condition to find differences. Researchers analyzed genetic information from over 1,500 people. They also looked at blood, skin cells, and DNA from smaller groups of patients and healthy people.

Six of the seven genes are involved in immune cells or inflammatory signals. This means that several genetic risk factors might work together through similar biological pathways.
Caroline Ran, an associate professor at Karolinska Institutet, noted that this supports the idea that the immune system and inflammation are key to cluster headaches. She explained that many of the genes affect the same signaling pathways, which gives important clues about how the condition works.
Traces of Tobacco Smoke
A second study explored a possible environmental link. Smoking is more common among people with cluster headaches. So, researchers looked for biological signs of exposure to substances found in tobacco smoke.

The team studied blood, DNA, and cerebrospinal fluid from patients and healthy people. They found that people with cluster headaches had markers linked to more exposure to heavy metals and other parts of tobacco smoke. They also had differences in markers related to DNA methylation. This is a chemical process that helps control gene activity without changing the DNA itself.
Andrea Carmine Belin, an associate professor at Karolinska Institutet, said that while the study doesn't prove cause and effect, it suggests environmental factors like heavy metals in tobacco smoke might interact with biological processes in cluster headaches.
Important Clues for Future Treatments
Together, these studies suggest that cluster headaches might involve more than just the brain's pain circuits. Immune signals, inflammation, environmental exposure, and how genes are controlled could all be part of a larger biological network tied to the condition.
More studies are needed to confirm these findings and understand how genetic and environmental signals connect. If confirmed, these discoveries could help researchers find biological pathways to target for new diagnostic tools or treatments.
