For a while, ketamine's use in older adults with severe depression has been a bit of a question mark. Concerns about safety and effectiveness in the 55-plus crowd meant it wasn't widely explored, despite its growing success in younger folks who haven't responded to other treatments. Because apparently, even our brains get set in their ways as we age.

The team at Texas A&M's EPMAD Lab, led by Professor Sanjay Mathew, is tracking how the brain reorganizes itself hours and even days after a single ketamine infusion. Krisha Shah, a research associate, notes that the brain is a highly connected system, and ketamine's unique trick is that its effects can linger long after the drug itself has left the building.
To really see what was happening, they used something called "high-order interactions" – basically, looking at how information is organized across many brain signals all at once, not just two. Think of it as mapping the entire river system, not just one tributary.
What they found was fascinating: ketamine didn't just create one new, stable pattern. Instead, the brain's organization kept shifting. Different patterns emerged at one hour, 24 hours, and seven days post-treatment. And here's the kicker: the greater the increase in brain-wide information sharing at the 24-hour mark, the better the improvement in depressive symptoms by day seven. Let that satisfying number sink in.
This research builds on earlier work that showed intravenous ketamine is actually both possible and effective for older adults. Now, by mapping these brain connectivity patterns, they're identifying potential "biomarkers" – specific brain signal changes that could tell us why ketamine works its magic in this demographic.
Murphy points out that most drug studies historically hit the brakes at age 65, leaving a huge gap in our understanding of the aging brain. Which is a bit of an oversight, considering older adults make up over a third of the US population.
While ketamine has come a long way from being just an anesthetic with a wild side, it still has potential for abuse. The goal? To pinpoint the exact neural switch points so they can develop new treatments that offer all the brain-flexing benefits without the psychedelic journey or other medical side effects.
